Just print out the form and bring it with you to one of the registration nights, or mail it to the P.O. BOX listed on the form. But remember, there will be a late fee charged if registrations are handed in after January 4th, 2012. 

Note: All travel team players must wear the LSC uniform.  For all new players the uniform can be purchased for $15 dollars on registration night or send in a uniform order form along with your registration form and payment using regular mail.

Any player looking to replace a portion of the uniform can download the uniform order form, fill in the appropriate areas and return the form with payment using regular mail.

Remember to Donate!  Bring all unwanted, outgrown soccer equipment to registration night and add to the LSC donation box. Equipment swaps are a great way to help families save money on athletic equipment.
